This application will take about 10 minutes to complete and all the benefit outlines are provided for your convenience in the link.   Completion of the application now is to obtain a quote and not to actually enroll in the program.  If you want to, you may scroll past the benefit outlines to the bottom of the page and “click to start application”.  Once we have your rates we can review programs options to see what may make the most sense for you.  We continue to offer four traditional programs, two HSAs and a Classic Value HDHP program.  In the application link you will be asked to select a program you might be interested in, and you may select any program to receive a quote on all of the programs.  Once you click the submit application button, you will receive confirmation the application is submitted.  If you don’t receive that please scroll back up into the application to see what question you may have missed.

In the bank information section you may put any bank routing number as these are public information, then in the bank account section please put 1010101 to fill up the field.
